5/28/2023 EnchantaVaughn Approved 1.Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Kaila expressed to QMHPA that she was having a good morning. QMHPA informed her that this session was going to focus on her IADLâs and ADLâs. Kaila did not present any symptoms of S/I or H/I at the time of the session. 2.Behavior: Kaila at times was observed being assertive mood when discussing triggers to her anxiety and depression. 3.Behavior: Kaila appeared ready for change when discussing ways to increase her ADLâs and IDALâs as she reports she has been learning how to live life on her own due to her limited family support. 1.Intervention: QMHPA assessed Kaila for symptoms of S/I. QMHPA discussed with Kaila depression and anxiety. QMHPA explained to Kaila being able to identify signs of depression and situations that cause people to become depressed. QMHPA educated Kaila on situation that cause people to become depressed and how to deal with depression. QMHPA displayed with Kaila physical depression through role play. QMHPA engaged Kaila in a skills building activity. QMHPA informed Kaila that the purpose of this activity was to teach Kaila appropriate behaviors when feeling depressed and how to manage depression before self-destructing behaviors occur. QMHPA assisted Kaila with reflecting on past events that caused her to become depressed. QMHPA explained to Kaila about being able to identify her psychiatric triggers. QMHPA discussed with Kaila about her outburst at times can be inappropriate and encourage Kaila to be able to identify them when they occur. 2.Intervention: QMHPA discussed with Kaila the effects of isolative behaviors and how being isolated could cause her to slip into a deeper depression. QMHPA engaged Kaila in a discussion surrounding situations that causes her to become depressed or said and how she deals with the situation. QMHPA conducted a skills building activity. QMHPA explained to Kaila that the purpose of this activity was to demonstrate with to Kaila coping mechanism through roles play. QMHPA explained to Kaila to apply coping skills when she is facing anxiety or depression. QMHPA guided Kaila through breathing techniques, exercises, walking, writing exercises and verbalization techniques. QMHPA discussed with Kaila the importance of being able to express her feelings and displaying positive behaviors. QMHPA educated Kaila on independent living skills and being able to function as a normal adult. 3. Intervention: QMHPA discussed with Kaila about following daily living skills such as complying with medication regiments, bathing, changing clothes and hygiene regiments. QMHPA encouraged Kaila in maintaining organization throughout the community and home. QMHPA discussed with Kaila the importance of personal hygiene regiments and how it will improve her living skills when completed daily. QMHPA reviewed with Kaila cleaning regiments such as washing clothes, separating clean clothes from dirty clothes and organizing her area in her room (getting hot to prevent rodents and other insects from entering the home). QMHPA engaged Kaila in a skill building activity. QMHPA explained to Kaila that the purpose of this activity was to demonstrated proper cleaning techniques through role play. 1.Response: Kaila appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Kaila expressed to QMHPA that she is still learning how to deal with anxiety and depression when it comes to certain things. Kaila expressed to QMHPA that she just struggles with depression badly when she over thinks or when things she doesnât understand. Kaila was able to understand how depression causes her to experience boredom and engage in other behaviors. Kaila expressed to QMHPA that she knows that isolating herself at times is not something that she needs to do, but it is the best thing to do when it comes to her depressive state. Kaila said she will listen to music when depressed or walk around. Kaila said she just feel like walking when questioned about her mental illness. 2.Response: During the skills building activity, Kaila was able to provide QMHPA with ways that she deals with depression and anxiety; however, she admitted to QMHPA that she doesnât apply those skills at all. Kaila expressed to QMHPA that she struggles with her independent living skills because she doesnât live on her own. Kaila expressed to QMHPA that she really didnât know a lot of independent living skills until she came connected with QMHPA. She discussed with QMHPA her mental health has a lot to do with it. Kaila was quiet when asked about personal hygiene. Kaila admitted does not practice proper hygiene regiments daily; however, she is a work in progress with it. 3.Response: Kaila reports she is not mentally prepared to complete the interview on July 5 as she does not have all the information they are going to need as it relates to her obtaining her SSI and for her children. She reports feelings as though she wants to give up but she knows that she has to continue to keep fighting because she is not the only one going through this. Kaila reports she will ask to rescheduled the appointment for later on in the week and hopefully she can get a quick appointment. She reports fear of rejection when it comes to different situations such as this one but she really needs to get it for all of her children and herself.
